CORE SERVICE AREA
Community Health Works' service area covers 2,233 square miles in which more than 350,000 people reside. Recent studies of the seven county service region (Bibb, Crawford, Houston, Jones, Monroe, Peach, and Twiggs counties) depicts that health status in the region is growing progressively worse. The uninsured and rural residents of the service region are among the state's most medically underserved and disadvantaged. Poor health status and the prevalence of chronic disease, combined with barriers to accessing healthcare, create a growing population of rural Georgians who are also becoming more ill. Morbidity and mortality due to chronic disease are higher for the uninsured, and those rates increase as economic status and access to care decrease. CENTRAL GEORGIA CANCER COALITION Beyond our core seven-county service area, the Central Georgia Cancer Coalition, a focused initiative of CHW, serves a twenty-five county service area that includes the seven core counties as well as Jasper, Putnam, Baldwin, Wilkinson, Laurens, Treutlen, Montgomery, Wheeler, Telfair, Dodge, Bleckley, Wilcox, Pulaski, Dooly, Macon, Upson, Lamar and Pike Counties. GA- HITREC Inititaive Our health information technology goals, in part fulfilled through the GA-HITREC (Georgia Health Information Technology Regional Extension Center) initiative supported by CHW to implement and integrate use of electronic health records throughout the central Georgia region, is focused on both public health district 5-1 and 5-2. STATEWIDE SERVICES Finally, Community Health Works continues to seek opportunities to extend our reach into communities across the state of Georgia, further impacting health outcomes for a broader segment of the population. One such initiative with a statewide reach includes the newly-developed Georgia Health Center Development Incubator Program. This program, created in June of 2010 in partnership with the Georgia Department of Community Health, allows CHW to provide hands-on training and technical assistance to facilitate, incubate, and otherwise support the development of community health centers or Federally Qualified Health Centers in communities across the state of Georgia. |
