Community Health Works’ goal to expand the number of primary care access sites and community-based health centers in the region evolved from our early work incubating First Choice Primary Care, an FQHC located in Bibb County. First Choice provides high quality primary and preventative services to the medically underserved, defined as anyone in need of a medical home, both insured and uninsured alike.
